The following information is required of applicants requesting support from the FLEX-able fund. Only funding support requests for Facioscapulohumeral Muscular Dystrophy research projects will be considered.

PROJECT TITLE

Provide a concise title to your work.

FUNDING REQUESTED

Please provide us the exact details of the funding you are requesting. For example, if you are requesting funds in order to hire a temporary laboratory assistant, provide us the details regarding hours required, rate of pay and length of employment needed or if funds are needed to support a research fellow stipend please provide necessary paperwork. If you require a new piece of equipment, provide the item, manufacturer and price of that item in your request. Please do your price evaluations and research prior to submitting your request so that you can provide us with the exact figures.

SCIENTIFIC ABSTRACT

Describe your research project for which these funds will be utilized. In 200 words or less provide us with the scientific description of your FSH research project.

LAY ABSTRACT

Briefly describe your research in 200 words or less using non-technical language.

In both abstracts, the researchers should clearly illustrate the significance of their research to the field of Facioscapulohumeral Muscular Dystrophy.

BIOGRAPHICAL SKETCH

A brief biographical sketch of requesting personnel must be submitted, this should be no longer than one page and should include education, professional and research background.

AFFIDAVID

A personal statement that these funds are not present in any other available funding, that no other grant funding which they have been awarded covers this particular need of their laboratory and for this research project. That this will NOT be a duplication of your current funding.

There are neither deadlines nor time restrictions to submit an Application for a FLEX-able Fund grant.
